FGS Important Dates

Graduate studies consists of three academic terms: Fall, Winter and Summer.  Use the FGS Important Dates to ensure you are aware of their deadlines.

FGS Advancing Master’s to PhD Form

Use this form if you are completing your Master’s degree and would like to continue your studies in the PhD program; if you are advancing to the PhD program without completing your Master’s degree or if you would like to start your PhD program while being dual registered for one term of your Master’s degree.

FGS Course Transaction Form

Use this form if you need to add, drop, audit or change a grade after posted deadlines.

Use this form if you would like to withdraw from your graduate program.

FGS Request to Take a Course in another Graduate Program at York

Use this form if you would like to enroll in a graduate course outside of your program.  For example, you would like to enroll in a graduate course in Mechanical Engineering at York.

Use this form if you would like to enroll in an upper year undergraduate course at York.

Use this form if you would like to take a graduate course at an Ontario University.  Note: This course must be a requirement of your program.  Please see eligibility requirements.

FGS Graduate Academic Petitions Portal

Use this link to submit your academic petition via the portal.  Per FGS, acceptable petitions include reinstatement to continue with three terms (12 months), extension of program time limits, course extension (for incomplete coursework), course drop (withdrawal), transfer credit (advanced standing), etc.

Use this petition form for any retroactive or other petition that the portal does not accept.

Use this form if your petition was not successful and you would like to appeal the decision.

ESS Graduate Program and Faculty of Graduate Studies (FGS) – Proposal, Thesis and Dissertation

ESS Research Proposal Form (find this form in the ESS Graduate Student E-Class)

Complete the ESS Research Proposal Form. Complete questions and upload the form to the Graduate Proposal Milestone Portal.

ESS Supervisor/Supervisory Committee Form (find this form in the ESS Graduate Student E-Class)

The supervisor should complete the ESS Supervisor/Supervisory Committee form in consultation with you, the student, and proposed supervisory committee members. The information on this form will be entered into the FGS portal by the Graduate Program Assistant. Faculty members will agree and approve membership via workflow within the system.

ESS – Recommendation for Oral Examination Form (find this form in the ESS Graduate Student E-Class)

The supervisor should complete this form in consultation with you, the student, supervisory committee and proposed examining committee member(s). The information on this form will be entered into the FGS portal by the Graduate Program Assistant. Note to supervisors: Please ensure that there is sufficient time between submission of form and the examination date.  If there is not sufficient time, the portal will not permit the Graduate Program Assistant to move forward with next steps and a new date will need to be selected.

Oral Examination Report Form (look for the form under Thesis and Dissertation in the Supervision section.

This form was emailed to the Chair of your examining committee. This form should be completed after your exam and immediately emailed to the Graduate Program Assistant.

Revisions Approved Memorandum Form (look for the form under Thesis and Dissertation in the Supervision section.

This form was emailed to you, the student, from the Faculty of Graduate Studies prior to your examination. If you have revisions, please complete this form and acquire your supervisor and examining chair’s signatures. In most cases, the examining chair will email this form to the Graduate Program Assistant who will then upload the form to FGS.

Delay of Publication (Embargo)

Upon completion of your upload, your thesis or dissertation will be uploaded to York’s repository.  If you require a delay of publication, please complete the FGS form.  You may be required to provide supporting documentation.
